To make your first visit as smooth as possible with no waiting and filling out forms in our reception room, our team will text or email (your choice) your New Patient Intake Form to you in advance.

This process saves you time.  You can even enter your insurance information, accident info if you are seeing us due to an injury and your ID information all ahead of time. This also allows us to check on any out of network insurance benefits you may have so we can be prepared to give you all the information you need to make informed decisions about your treatment.
