Scheduling an appointment is easy! You can book an appointment here on our website, call or text us at 708-456-8844. After confirming your appointment, we will ask how you prefer your new patient paperwork and intake forms sent to you, via email or text. Once you receive the link to fill out your paperwork, it only takes a few minutes to fill out and this will save time when you arrive in the office and we will be able to get you started right away!

Don't worry if you are unable to fill out the paperwork ahead of time. While it saves you time and makes your visit more efficient, we can also assist you in filling it out in the office!

At Smith Chiropractic Health Care, we believe in empowering you with knowledge about your health. After your initial examination, we will spend some time reviewing your findings.  On your second visit, we will provide a personalized Report of Findings giving you a clear, detailed breakdown of what we’ve discovered and how we can help you feel your best through Chiropractic.

This report will include recommendations for treatment both in our office and at home.  Your treatment plan is a team effort and we believe greatly in the doctor-patient relationship.  We want to work with you on your health in a way that is comfortable for you.  We will share our ideal recommendations with you on the best way to get healthy and feeling better, but ultimately your health is in your hands and we would love to be part of your journey back to health!

We will also review the cost of your treatment with you so you can make the best health decisions for you and your family!

Once you begin treatment at Smith Chiropractic, most patients find they respond relatively quickly where their pain and symptoms are concerned.  However, in order to provide the best customized care to you, we will do periodic assessments to measure your progress and modify your treatment plan as needed.  This includes a chat with the doctor about how you are feeling, a comparison of your posture analysis, range of motion tests and any other tests that are needed.  We will then review your findings with you as well as provide any home instructions and continued care recommendations!

You will be very kindly greeted with a smile by our front desk assistants, given a tour of our wellness facility, and escorted to meet with the doctor in a consultation.

Our doctors are equipped with state-of-the-art equipment to find the root cause of your health concern, so rest assured, you and your family are in the right place! On your first day, you will receive an in-depth Consultation, a Chiropractic Exam, Posture Analysis, low dose Digital X-rays (if necessary) and a Chiropractic Treatment. Immediately, we will schedule you for your second visit to review the findings from your initial visit.
